Collect information and files with a form
Start from a template, add file upload questions and send one link. Answers and uploads stay in the client's workspace.
Forms collect structured information, such as a new client's details or a service request. A form can also collect files, like a network diagram or an insurance card.
Start a form
- Open the client's workspace. A form belongs to the workspace you create it in, and its answers stay there.
- Go to Forms and start a new form, or start from a template.
- Edit any question, or add fields from the field list: text, email, phone, number, long text, checkboxes, radio buttons, dropdown, date, time, address and File upload.
To use the same form for another client, copy it into that client's workspace.
Add a file upload question
- Add a File upload field.
- Set its Label, and whether it's Required.
- Under Accepted files, choose Documents, Images, Spreadsheets, or a mix.
- Set the Max file size. It's 25 MB by default, and you can raise it to 100 MB per file.
Each upload field takes one file. Add more upload fields if you need several files, for example the front and back of an insurance card.
Where answers and files go
- Answers are saved in the form's workspace.
- Uploaded files go to Files in that workspace, under Form uploads, in a folder named after the form. Each response links to its files.
- Files uploaded through forms are scanned for malware. A flagged file can't be opened or downloaded, and the owner is alerted.
Keep sensitive details out of forms
Don't ask for passwords in a form. Use Secure Drop for passwords and codes, and send agreements for signature with Sign.
Common questions
Which file types can clients upload?
Documents (PDF, Word, text, RTF, ODT), images (JPG, PNG, HEIC, WebP, GIF) and spreadsheets (Excel, CSV, ODS). You choose which of these each upload field accepts. Web pages, scripts and SVG files are never accepted.
Do uploads count toward my storage?
Yes. Form uploads count toward your account's storage. A response counts as one form submission, however many files it carries.
Does a form submission create a new workspace?
No. Answers and files always go to the workspace the form belongs to.
